AD/HD Evaluation
This information provides a common understanding and knowledge base of the components that are necessary to assess for AD/HD, the impact of AD/HD on the individual’s functioning, and the possible need for accommodations.
Documentation
If you have not already done so, a complete history of the person to be assessed must be completed. Please provide records or documentation that you believe would be helpful in assessing for AD/HD to the assessing clinician. Here are some suggestions:
- school
- teachers
- pediatrician
- daycare
- preschool
- psychiatrist
- psychologist
- therapist/counselor
- supervisors/managers
A clinical interview may be required.
Testing
Testing consists of multiple questionnaires that are completed by a number of people who can provide information about the person being assessed. Usually, links to the questionnaires are emailed and the person will complete it online. However, the questionnaires can be given in other ways if necessary. Your clinician will work with you to find the most appropriate way of getting the information needed. Here are some suggestions for appropriate people who can complete a questionnaire. In most cases, the person to be assessed will also complete a questionnaire.

- family members who have spent significant time with the person to be assessed

Report
Once all information is gathered and the questionnaires are completed, the results will be compared to the DSM V Diagnostic Criteria for AD/HD. A complete report will be provided along with a diagnosis if applicable.
Fee
The cost for an AD/HD evaluation is $500. Insurance does not cover this evaluation, and $250 is required at the time the evaluation is requested and the remaining $250 at the time you are provided with the final report.
The evaluation fee includes:
- BASC-3 Behavior Assessment System for Children 3rd Ed (up to 3 raters)
- Conners 4th Edition (up to 3 raters)
- Clinical interview with parent/caregiver
- Review of documentation and complete history
- Evaluation Report
- Parent meeting to discuss the evaluation results and recommendations
- Intervention suggestions for areas of concern
